Understanding Box 15 on your T4A

You may have received a Notice of Eligibility Review letter based on the dental coverage access information reported in Box 15 of your T4A. Health Canada reviews your eligibility for CDCP, which includes reviewing access to dental coverage.

What the code means

• The code in Box 15 of your T4A is correct
• The code indicates access to dental coverage through the Teachers' Pension Plan
• It does not reflect whether you were enrolled in the plan or eligible to enroll

Why the plan reports access

The Teachers' Pension Plan does not have information about any other dental coverage you may have.

If you have dental coverage through another comparable provider, you may still be eligible to enroll in the pension plan’s dental coverage. Because we do not collect information about your other coverage, you are considered to have access.

If you do not have dental coverage through another comparable provider and are no longer eligible to enroll, you are still considered to have access because we do not collect that information.

Why this is on your T4A

The Canada Revenue Agency requires the plan to indicate whether members have access to dental coverage through the plan at retirement. This is effectively reported each tax year.
