Get a proof of eligibility letter

If you received a Member Eligibility Review Form from Health Canada, you can use a letter from the pension plan instead of completing the form.

If you have access to My Account:

1. Sign in to My Account
   • Go to Statements
   • Select Canadian Dental Care Plan letter
   • Click CDCP letter
2. Save or print a copy
3. Send the letter to Health Canada

If you do not have access to My Account:

1. Email us
   • Send an email to retired@pensionsbc.ca requesting a letter that confirms you waived or cancelled dental coverage with the Teachers' Pension Plan before December 11, 2023
   • Include 3 of the following pieces of information:
     • Name
     • Address
     • Phone number
     • Date of birth
     • Person identification number (PID)
2. Receive your letter
   • We will email your letter to you within 14 days
3. Send the letter to Health Canada

Note: Do not send a Member Eligibility Review Form to us. The deadline for returning information to Health Canada is short.
