Submit a form to waive your death (survivor) benefit before retirement
You can give up your right to receive a pension benefit if you are the spouse of a plan member who dies before retiring.
Eligibility
Only complete Form 4 Spouse’s waiver of beneficiary right to benefits in a pension plan, locked-in retirement account, life income fund or annuity before pension or annuity payments start if your spouse (the plan member) has not yet started receiving their pension.
If your spouse is retiring and applying for their pension, you must complete Form 2 Spouse’s waiver of 60% lifetime survivor’s benefit and/or beneficiary rights from a pension plan or annuity after payments start to give up your right to a minimum 60 per cent lifetime survivor's (death) benefit or beneficiary right to a guaranteed monthly pension.
Choose one of two ways to submit your spousal waiver form.
Gather your spousal waiver information

Your witness must be of sound mind, at least 19 years old, not the plan member or any of the named beneficiaries of the pension benefit. Your witness must be present when you sign the form.

Submit your spousal information
Option 1: Complete the form using the digital signature tool
- If you choose to use this option, read these Terms and conditions as they apply to your use of the digital signature tool
- Provide the required personal information First and Last name, email address, for you (the spouse) and your witness to receive a secure link to the Spousal Waiver form
- Complete the form sent to the email address you provided
If you haven’t received the email within 10 minutes, check your junk/spam folder - Initial the first box in the waiver section to give up your beneficiary right to receive your spouse's pension benefit if your spouse dies before retiring
- Digitally sign and submit your form
Note: Your witness must watch you sign the form and your spouse (the plan member/former member) must not be present - The form will automatically be sent by email to your witness to sign digitally
- Once signed by your witness, the form will automatically be sent to us to process
We will send you and your spouse a confirmation message once we have updated our records with your spousal waiver form.
Option 2: Submit the paper form
- Download and print Form 4 Spouse’s waiver of beneficiary right to benefits in a pension plan, locked-in retirement account, life income fund or annuity before pension or annuity payments start
- Initial the first box in the waiver section to give up your beneficiary right to receive your spouse's pension benefit if your spouse dies before retiring
Your spouse (the plan member) must not be present when you complete it - Sign the form in the presence of a witness
- Have your witness sign and date the form; they must sign on the same day
- Mail or fax a copy to us
If your spouse is submitting a Nomination of Beneficiary (pre-retirement) form, give them the Spousal Waiver to submit with their beneficiary form
