If you have a spouse
If you have a spouse, they are automatically your beneficiary. If your spouse wants to give up their beneficiary right to a death benefit, you must submit Form 4 Spouse's waiver of beneficiary right to benefits in a pension plan, locked-in retirement account, life income fund or annuity before pension or annuity payments start. Once the plan has this form on file, you can sign in to My Account to name a new beneficiary or change your beneficiary to someone other than your spouse.
If you are a limited member
Submit the Nomination of beneficiary (for limited member) form.
